*New patient appointments can take anywhere from 45-60minutes
New patient paper work can be completed prior to the arrival at Mommy & Me Family Chiropractic by going to the New Patient tab on our main page and clicking on the appropriate forms.
If you wish and it is more convenient, you can come to the clinic 15 minutes prior to your scheduled appointment and fill them out in our office.
Once the paper work is completed, Dr. Carly will then take a health history and address any concerns or specific questions you may have.
If Dr. Carly thinks that chiropractic is a good choice for you at this time, she will then perform a hands on exam which will include performing specific orthopedic testing, neurological testing, postural examination, range of motion and motion palpation.
*Some cases Dr. Carly will require the patient to have x-rays take prior to treatment. Dr Carly does not take x-rays in her clinic, if she thinks that it is important for the patient to have x-rays prior to treatment, she will then refer the patient out to get them taken. Once x-rays are completed Dr. Carly will contact the patient to schedule an appointment to go over the results and discuss a treatment plan.
If x-rays are not required a computerized thermography scan will then be performed. This scan helps to measure nerve stress to organ function.
Once Dr. Carly has completed a thorough history and exam she will then sit down with the patient to explain everything and recommend a treatment plan.
At this time the patient may decide to accept or decline care.
Mommy & Me Family Chiropractic does direct billing to private Insurance Companies.
Motor Vehicle Accidents
Mommy & Me Family Chiropractic accepts AUTOPAC claims.
It is the responsibility of the patient to make their own AUTOPAC claim. Once the claim is made, AUTOPAC will give an injury claim number, which is separate from a vehicle claim number. Once you have been given an injury claim number you may make an appointment to see Dr. Carly.
Dr. Carly will be responsible for all paperwork and submission. Once approved, patient care may begin.
Direct billing will go straight to AUTOPAC and the patient will not be responsible for any payments that are given for treatment through their injury claim number.
* All ages may be given an injury claim number if involved in the accident. It is recommended that children also be checked post accident to prevent future problems to the spine and nervous system.
Work Related Injuries
Mommy & Me Family Chiropractic accepts work related injuries (WCB). It is the responsibility of the injured employee to make an injury claim with their employer. The employer is then responsible for filling out an Employee Accident Report and then submitting it to WCB.
Dr. Carly is considered a primary care practitioner as long as the worker has not visited a Medical Doctor prior. If the worker has seen a medical doctor, a referral note is required.
Dr. Carly will fill out and submit a chiropractors First Report to WCB outlining the workers injuries. Upon approval from WCB, chiropractic care may begin and will last the time frame WCB has approved.
